Pediatric Social Worker

HealthCare Support is actively seeking a Pediatric Social Worker to fill an opening with a Fortune 50 Managed Care in Lee county FL.

Daily Responsibilities for Pediatric Social Worker:

• Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical staff as necessary to meet the complete medical socio-economic needs of clients
• Provide patient and provider education
• Facilitate member access to community based services
• Monitor referrals made to community-based organizations, medical care and other services to support the members’ overall care management plan
• Monday to Friday, 8am to 5pm (No OT required)/Hybrid
• 3 month contract starting out- may be extended and strong potential to convert to permanent.
  

• Must have a MSW and 1 year of Pediatric experience or LPN with 4+ years of Pediatric experience
• Must have a Valid and Active Driver’s license
• Must have MS Office exp - Excel, Outlook, Word
• MUST be located in Lee county

• 2 years of PEDs exp
• 1 year of PEDs exp and 1 in general social work exp
• Experience with EMR
• Case management
• Bilingual in Spanish highly preferred

Pay Details: $27 - $32.69 an hour
