Job Title: Clinical Pharmacist
Location: Ardmore, OK
Setting: Hospital
Hospital Size: Licensed for 190 beds
Schedule: Monday to Friday day-shifts, with occasional weekends (Flexibility available)
Start Date: ASAP
Duration: 13 weeks or more (Temp-to-hire opportunity)
Responsibilities
1. Medication Order Verification: Review and validate medication orders to ensure accuracy, appropriateness, and adherence to formulary guidelines.
2. Patient Medication Counseling: Provide prehensive medication counseling to inpatients, including dosage instructions, potential side effects, and medication administration techniques.
3. Pharmacokinetic Monitoring: Perform therapeutic drug monitoring and adjust medication dosages based on patient-specific factors such as renal function, hepatic function, and drug interactions.
4. Collaborative Interdisciplinary Care: Work closely with physicians, nurses, and other healthcare professionals to develop and implement patient-specific medication therapy plans, ensuring optimal therapeutic outes and patient safety.
Ideal Candidate:
- Active Oklahoma pharmacist license, required
- Prior hospital experience preferred
