Contract Home Health Registered Nurse (RN)-Holland, OH

Position Details:

• Type: Full-Time Contract
• Location: Holland, OH
• Hours: 40 hours per week, Monday to Friday, 8:00 am to 5:00 pm
• Schedule: Occasional weekends and on-call duty, typically once per month
• Compensation: Pay negotiable based on experience and territory budget
• Benefits: Medical, dental, vision, 401k available after 90 days
• Additional: Stipend available for travelers, mileage reimbursement visit to visit

Requirements:

• Active Ohio Registered Nurse (RN)license
• Active American Heart Association or American Red Cross Basic Life Support (BLS) certification
• Minimum 1 year of home health experience
• Minimum 2 years as a Registered Nurse (RN)
• Proficiency in Oasis and Homecare Homebase software (training will not be provided)
• Reliable transportation

Job Duties:

• Perform comprehensive assessments of patients' health needs and develop personalized care plans
• Deliver skilled nursing care and treatments in patients' homes, including wound care, medication management, and patient education
• Document all patient care activities accurately and promptly using Oasis and Homecare Homebase software
• Collaborate with interdisciplinary team members to ensure coordinated care and optimal patient outcomes
• Adhere to all regulatory guidelines and agency policies to maintain quality care standards
