Case Manager / LMSW
HealthCare Support is actively seeking a Remote/Field Case Manager / LMSW to fill an opening with a Healthcare Organization in Pinal County, AZ!
Daily Responsibilities for Remote/Field Case Manager / LMSW:
- Collaborate with the core functional care team to provide psychosocial interventions and case management services to a patient panel
- Interact with and evaluate patients in the home, via teleconference, or via telephone as appropriate
- Identify and manage patients’ psycho-social conditions and obstacles to care
- Shift: Mon-Fri 8am-5pm (hybrid schedule)
- Licensed Clinical Social Worker (LCSW) in the state of Arizona
- 3+ years of experience as a Social Worker/Case Manager supporting the geriatric population
- Experience supporting patients with chronic illnesses

Pay Details:
- $70-85K DOE – Flexible based on experience
- Eligible for up to 10% annual bonus!
