We are looking for a Part Time Medical Social Worker to join our team!

WORK FROM HOME with exception of patient appointments (done every 6 months) and occasional home visit check ins.

Hallway Healthcare provides in-home nursing care to retired Nuclear and Federal Workers that have received benefits through EEOICPA or FECA. These are the workers who have retired from Department of Energy facilities such Lawrence Livermore National Lab, Sandia National Lab Lawrence Berkeley National Lab, Stanford Linear, etc. We also serve Federal Workers such as US Postal Workers.

PAY: $32-$40/hour

Requirements:

Bachelors Degree

1 year experience in Medical or Home Health setting

Medical Social Worker/Case Manager Duties:

Interview clients to gather information about their backgrounds, needs, or progress

Collaborate with other professionals to assess client needs or plan treatments

Supervise and oversee all aspects of care and documentation for Licensed Nurses, PCA’s, and HHAs

Coordinate with skilled nurses to ensure that accurate and clear documentation of care is tied to the POC and DOL approved Diagnosis

Daily monitoring of comprehensive care in order to ensure and maintain the highest standard of care and remain in compliance with DOL policy

Perform monthly assessment and summary of patients condition-Supervisory visit of LVN/HHA, PCA

Ensure CARE COORDINATION NOTES AND SNV REPORTS along with case management report updates for the last six months are presented to the Medical Director.

Provide prepared LMN, Plan of Care, and 17B at initial and RENEWAL OF CARE exam for review and signage by treating physician.

Assess all aspects of care and services are in compliance with all rules and regulations

Supply ordering from various vendors for care of the patient, reorder as needed

Coordinate with skilled nurses to review/update any medications, provide education of medications related to DOL covered conditions, notify MD of any refills, need for change, or new medications.

Review/update Plan of Care, coordinate with skilled nurses to educate patients of Plan of Care related to DOL covered conditions and notify MD of any changes.

Provide instruction/verbal orientation to ALL new nurses, patient, caregiver, and staffer of DOL covered conditions.

Assess patient’s compliance with the Plan of Care.

Collaborate with MD on any needed or new referrals, treatments or durable medical equipment.

Perform supervision and staffing of all staffers in relation to patients needs and within DOL approved hours.

Discuss in conference with patient, caregiver, and staffers of patient’s DOL covered conditions status, changes, updates, request, referrals, or concerns

Collaborate with scheduler regarding patient preferences, assist with staffing needs/call offs if needed

While patient in hospital-daily calls to hospital case manager to inform of patient having home care and update HH staff on patient condition and discharge plan

When patient returns home from hospital-in home coordinate assessment within 48 hours

I understand the above and have all the tools to complete the tasks for each of my patients. I have been provided with training to adequately complete the above tasks for my caseload and anticipated caseload.
