Position Highlights *Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being. This includes educating staff on the role of social services. *Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs. *Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life. *Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers. *Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
