Responsibilities:

Bon Secours Medical Group is currently seeking a dedicated and experienced Nurse Practitioner to join our Primary Care team in Port Jervis, NY. The successful candidate will play a crucial role in providing comprehensive and patient-centered care to individuals in our community.

Key Responsibilities:

Patient Care:

• Conduct thorough assessments, diagnose, and develop treatment plans for patients in a primary care setting.
• Administer appropriate interventions, medications, and treatments as necessary.

Collaboration with Medical Team:

• Work collaboratively with physicians, nurses, and other healthcare professionals to ensure integrated and coordinated patient care.
• Participate in regular team meetings to discuss patient cases and treatment plans.

Shift Schedule:

• Work a 10- hour shift Tuesday-Friday or Monday-Thursday schedule to provide consistent primary care services.
• Be an essential part of our weekday operations to address the healthcare needs of our community.

Weekend On-Call Rotation:

• Participate in a rotational on-call schedule, with occasional weekends on-call.
• Provide telephonic support and coordinate with the medical team for urgent patient concerns.

Qualifications:

• Current Nurse Practitioner license in the state of New York.
• Board certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
• Strong diagnostic and clinical skills with a focus on preventive care.
• Excellent communication and interpersonal skills
