Essential Functions Statement(s)

  • Must be able to perform all duties expected of a Registered Nurse in a long term care setting
  • Utilizes established standards/criteria to oversee resident care services
  • Assesses, plans, evaluates the implementation of community and UCH programs, policies, procedures and forms
  • Assesses, plans, evaluates care delivery systems, by monitoring resident outcomes, compliance with Quality Improvement Standards and State, Federal, local regulations and other agencies
  • Assesses, plans, evaluates resident care delivery in meeting family needs and addressing concerns
  • Maintains an "on call" status for emergencies on a rotating basis with other supervisory nursing personnel
  • Oversees the professional nursing staff in the development, coordination and implementation of the resident's treatment plan
  • Oversees the proper execution of the following: NG/tube insertion, placement and irrigation; Colostomy care (including irrigation); Foley catheter care (including insertion and irrigation); Texas catheter application; Straight catheterization; Tube feeding, including hanging and labeling; operating the pump; calculating intake and output
  • Ensures dietary recommended feeding allowances are maintained and monitors the weights of residents, assessing for abdominal distention and residual tube placement
  • Administers proper assessment skills and techniques as appropriate, including the following: Gastrointestinal, genitourinary, pulmonary, cardiac, circulatory, neurological, musculoskeletal, and integumentary assessments
  • Administers proper positioning techniques: Transfer techniques, ROM exercises
  • Administers emergency procedures as necessary, including properly administers oxygen, suctions, performs CPR, and uses the ambu bag
  • Demonstrates expertise in the care of the patient with a central line
  • Complies with the State regulations specific to RNs in the insertion, administration and care of IVs
  • Demonstrates expertise in the care of the patient with hyper alimentation
  • Demonstrates knowledge of and monitors for potential complications related to central lines, and PICC lines
  • Consults with dietician regarding the special dietary needs of the residents and follows through on dietary recommendations, as necessary
  • Oversees care of residents with significant weight loss/gain and identifies at risk for nutritional problems and implements preventative measures
  • Reviews medication documents, treatments and prn's as well as completed 24 hour report and takes action, as appropriate
  • Oversees accuracy of all residents charts according to policies and procedures
  • Interprets policies and procedures to personnel, residents, visitors, and government agencies as required
  • Understands resident's rights and conducts (as appropriate) investigations related to any alleged abuse
  • Ensures progress notes reflect the care plan and that approaches on the care plan are followed and that assigned nursing assistants are aware of and carry out the resident care plans
  • Assesses, plans, evaluates the effectiveness of nursing interactions with other departments and support services
  • Participates in assessing, planning, and monitoring implementation and evaluation of each residents' interdisciplinary plan of care, as well as discharge planning and service coordination
  • Promotes good working relationships with community agencies and with all community departments
  • Participates in community health programs, as requested
  • Ensures all company policies and procedures reflect current practice
  • Communicates trends and issues in care delivery to the Administrator
  • Participates in planning, developing, and instructing nursing staff on the job orientation and staff development programs
  • Oversees Quality Improvement reviews, validating activity trends and monitoring infection control practices
  • Ensures the department is properly staffed at all times, and encourages employees to understand their role in the organization
  • Monitors the general performance of staff and recognizes and rewards good performance, or recommends training or disciplinary intervention if necessary
  • Sets individual and departmental objectives that are aligned with the organization's overall strategic objectives
  • Creates an environment encouraging excellence and sets a good example
  • Follows all legal and best practice Human Resource management guidelines and performance management tactics
  • Gives employees sufficient authority to make the necessary decisions to carry out their assignments
  • Identifies and maintains accountability for security within assigned areas of responsibility, ensuring employees understand their obligations to protect company and employee property and takes appropriate action when security or safety infractions occur
  • Investigates and responds to any employee grievances received
  • Maintains confidentiality of necessary information
  • Attends and participates in staff, departmental and other community meetings and sits on required committees
  • Assists in developing and updating departmental procedures and policies
  • Maintains required records and reports as outlined in UCH's Policies and Procedures manuals
  • Participates in department budget planning and generates monthly report, as requested
  • Handles unusual occurrences calmly and logically to maintain continuity of business and duties
  • Maintains appropriate, effective communication with residents, and with residents' family or other significant relationships, and ensures departmental staff does the same
  • Follows all appropriate safety and security guidelines, care procedures and protocol for residents in the Dementia Care Unit
  • Participates in relevant educational and training activities as appropriate
  • Performs nursing-related administrative duties (i.e., completing medical forms, reports, evaluations, studies, charting, auditing medical records for documentation of compliance issues, etc.) as necessary
  • Participates in special activities as required
  • Performs all other duties as assigned or directed

Skills & Abilities

  • Education: Bachelor's Degree (four year college or technical school): Preferred
  • Experience: Must have at least four (4) years" experience in long-term health care, with two (2) years of prior nurse management experience
  • Computer Skills: Must be proficient in using a computer including Microsoft Office (Excel, Word, etc), creating spreadsheets, the Internet and basic office equipment.
  • Certifications & Licenses: Successful completion of a state-approved pre-licensure nursing education program; Active RN license conferred by the State Board of Nursing; Must obtain and maintain mandatory, state and federal requirements and certifications for practice or occupation; Must participate in Continuing Education; Must be CPR certified
  • Other Requirements: Must be able to read, write, understand and speak the English language; Special care or dementia care employees will have additional training; Must be able to work with elderly residents, families and other health care staff members; Must be able to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook
