CareLink is a non-profit network of senior care providers working together to ensure that adults in our community can access the services and supports they need at the times when it matters most. CareLink provides multi-disciplinary rehabilitation services at Saint Antoine Residence - a 200-bed skilled nursing and rehabilitation facility located in North Smithfield, RI. The rehab facility at St. Antoine includes the “Easy Street” simulation of a city block including a modified Honda Accord, ATM, shopping store fronts, and a variety of city-like walking surfaces so rehab patients can practice everyday skills in a safe, therapeutic environment.
We are looking for Physical Therapists to join our team at St. Antoine Residence for a part-time opportunity!
Position Overview: Physical therapists help patients with injuries and chronic health conditions regain their range of motion, manage their pain, and improve their quality of life. The Physical Therapist (PT) is responsible for evaluation, design and delivery of prescribed Physical Therapy programs (within the parameters of training and expertise) to clients and residents of the partnering facility.
Essential Functions:
- Fulfill all job functions of a staff Physical Therapist within the clinical scope of practice.
- Diagnosing patients by observing their movements and listening to their concerns
- Reviewing patients’ medical histories and develops an individualized treatment plan
- Outlining clear goals for patients and the expected outcomes of the plan
- Using exercises, stretching, equipment, and hands-on therapy to manage patients’ pain, increase mobility, and prevent further pain and injury
- Recording patients’ progress and modifying the plan of care if needed
- Educating patients and family members about the recovery process
- Responsible for initiating prescriptive care as appropriate.
- Communicating within a multi-disciplinary team who are committed to the patients’ well-being
- Responsible for documentation of these evaluations and treatments, recording patient status, treatment response, changes noted, patient/family education, recommendations and other patient-centered actions taken.
- Assists in the implementation of patient treatment, including overseeing Physical Therapist Assistant’s as necessary.
- Complying with federal, state, and local certification requirements.

Education/Experience Required:
- Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited physical therapist program
- Valid Physical Therapist license in the state(s) where services are rendered.
- Interest in working with all abilities including the intellectual and/or developmental disabilities (IDD) population
- Must possess a valid and current driver license and auto insurance.
- Current BLS and/or CPR certification from the American Heart Association or American Red Cross.
