Job Description
POSITION SUMMARY: Provides a variety of support services to homeless or near-homeless Veterans affected by mental illness; assists in coordinating services and communication among specialists and CNCAP; provides financial assistance for eligible persons; identifies and accesses resources; seeks to expand program and resources independently and confidently; promotes and maintains public relations; and completes forms accurately and timely. Provides case management, and assists in recruiting and enrolling Veterans into the H.E.L.P. program.
ROLE AND RESPONSIBILITIES
- Responsible for completing the agency intake form and inputting it into Clarity
- Provide ongoing support to veterans and document via home visits, phone calls, and written correspondence with goal attainment of 65% or above
- Maintain a non-judgmental attitude toward clients and other CNCAP staff
- Ensure clients are assessed through the grant process to determine possible assistance through resources
- Create family personal plans to develop short and long-term attainable goals with veterans
- Ensure files are complete with the necessary paperwork for veteran families participating in the program
- Adhere to all CNCAP-related policies and procedures
- Promote the program and agency in a positive and professional manner
QUALIFICATIONS
- High school diploma or equivalent required (Bachelor's degree in human services or related field preferred)
- Valid driver's license
- 1-3 years of experience in human services or related field
- Must be a Veteran with lived experience
SKILLS & ABILITIES
- Problem-solving skills
- Communication skills
- Knowledge of word, excel, PowerPoint, and databases
- Excellent time-management and organizational skills
- Demonstrated ability to work with others
SCHEDULE: Monday-Thursday: 7 am - 5 pm or Flexible
REPORTS TO: SSVF/SSG-Fox Coordinator
JOB STATUS: Full-Time, 40 hours per week
PAY: $19.50-$20.50 (depending on qualifications)
