Job Description

Southern IL Case Coordination Services provides conflict free case management to individuals who have a developmental disability.

The duties include but are not limited to:

  • Development of a comprehensive Personal Plan
  • Collaborating with service providers to ensure that the individual's health, safety, welfare and well-being is as desired and that the individual is satisfied with services received.
  • The ISC encourages individuals and families to make informed decisions, exercise choice, and have maximum control over their lives.
  • The ISC works from his/her home and is independent in scheduling required work duties and responsibilities.

Candidates must possess a Bachelor's Degree and a minimum of 1 year experience working with individuals who have a developmental disability. An RN with a minimum of 1 year experience working with individuals who have a developmental disability.

Job Type: Full-time exempt status. No over-time is available.

Exempt Salaried position: $44,000.00 per year with opportunity to earn 5% incentive pay. Starting pay can be negotiated based on experience, qualifications, QIDP status

  • On the road (visits to homes, providers, employment) Standard IRS rate for mileage reimbursement
