SUMMARY

Responsible for care coordination of Medication Assisted Recovery patients. Facilitates communication between MedLink providers, accountability court counselors, recovery care organizations, and MAR patients.


ESSENTIAL DUTIES AND RESPONSIBILITIES (include, but are not limited to, the following)

Medication Assisted Recovery (MAR) Coordination

  • Review patients in the clinic daily, participating in patient assessment and preparation, including day-to-day program concerns, and ensuring education and counseling has been completed by providers
  • Enrolling patients into the treatment program and scheduling initial assessment
  • Supporting the patient and provider in the program initiation, stabilization, and maintenance treatment procedures
  • Assisting members of the health care team in the plan of care
  • Assisting the program provider in making referrals to community providers for counseling or higher levels of care when needed
  • Maintaining therapeutic relationships with the patient and the provider
    • Communicates with providers, drug court counselors, clinical staff, and referral sources to coordinate care for MAR patients through the various levels of care.
    • Care coordination including medication and treatment monitoring
    • Insurance authorization and troubleshooting
    • Relationship building and patient linkage to additional support (drug treatment services and mental health care)
    • Relationship building and facilitation of ancillary services
  • Sharing information with other members of the health care team and evaluation team through chart documentation, interdisciplinary team meetings, and email
  • Coordinates MAR medication issues, including consent, authorizations, and administration of medication. Communicates with MAR Program Coordinator regarding such issues.

Pre-Visit Planning:

  • Working with the Provider, Practice manager, Practice Site Supervisors, Front Office Assistants, and Clinical Staff, to prepare for patient presenting to office for MAR initation and follow up.
  • If lab/diagnostic imagining or specialist referral is noted on the visit summary, obtains results if not in EMR.
  • Obtains reports if reason for visit is noted as hospital or emergency department follow up.

For MAR enrolled patients:

  • Obtains informed consent from patients prescribed by the provider
  • Reviews the medical record to determine care plan ordered by the provider
  • Provides patient counseling regarding medications, barriers to care, drug screening, follow-up appointments
  • Identifies and links patients with community resources to facilitate referrals and respond to social service needs.
  • Completes screenings for provider as needed
  • Reviews treatment goals with patients.
  • Provide relevant self-management support for enrolled patients
  • Assist patients with the process of determining eligibility, obtaining and maintaining pharmaceuticals

General Duties and Responsibilities

  • Establishes and maintains relationships with identified service providers and community resources
  • Demonstrates excellence in both internal and external customer service
  • Understands and effectively communicates HIPAA compliance, corporate compliance and client confidentiality
  • Ensures compliance with local, state, and federal regulations
  • Serves as a resource for center staff for MAR protocols and guidelines
  • All other duties as assigned

EDUCATION and/or EXPERIENCE

Licensed Practical Nurse Degree or Medical Assistant Certification. Prior experience working with patient with substance use disorder or mental health diagnosis. Completion of Medication Assisted Treatment Specialist certification or other similar certification preferred.
