Job Description

What You'll Do:

You'll be at the forefront of coordinating, developing, and completing all MDS Assessments with precision and expertise. From ensuring compliance with state and federal regulations to leading a team of RN MDS Coordinators, you'll play a pivotal role in shaping the care plans that impact residents' lives.

Key Responsibilities:

  • Coordinate and oversee the completion of MDS Assessments across all units, maintaining compliance with facility policies and regulations.
  • Lead daily PDPM meetings, ensuring accurate capture of clinical data and revenue optimization.
  • Collaborate with interdisciplinary teams to develop comprehensive care plans for each resident, setting measurable objectives for optimal outcomes.
  • Serve as a resource and educator, guiding MDS, Care Planning, and resident assessments.
  • Analyze CASPER reports to identify trends and opportunities for improvement, driving quality initiatives within the facility.

Qualifications

  • Minimum of two years of MDS experience
  • Registered Nursing program completion with current state licensure
  • AANAC Certification
  • Previous experience in older adult or retirement housing industry preferred
