Job Description
  • Participates as an active member of the interdisciplinary team, determines nursing goals of the care plan, and monitors the participant's progress toward goals
  • Provides skilled nursing care and prescribed treatments to participants in their home
  • Directs and supervises the provision of quality para-professional clinical care to participants in their home
  • Conducts customer service home visit assessments of each new participant within the first week of program enrollment and performs re-assessments and evaluations of each participant
  • Assist with referrals to other services when participant conditions or situations require the service of other professional disciplines or the products or services of outside companies or agencies

Qualifications
  • RN or BSN degree with current RN licensure in the Commonwealth of Pennsylvania
  • Current Pennsylvania Driver's License
  • Thorough knowledge of current community health nursing practice and skill in the application of current nursing procedures
  • Ability to effectively plan and coordinate nursing care for individuals, families and groups
  • Ability to work independently and as part of a team, under pressure, while maintaining cooperative working relationships
