Director of Social Work

Responsibilities of a Director of Social Services:

  • Provide oversite and direction/updates to the Social Services department.
  • Develop, implement, and evaluate department policies and procedures.
  • Interview, hire, and onboard new social service employees in collaboration with the Human Resources department.
  • Provide periodic performance evaluations of department staff.
  • Prepares and organizes the admission of new residents. This includes meeting with resident and/or family in a timely manner after admission, completing all necessary paperwork, distributing paperwork, addressing resident/family concerns as they adjust to facility.
  • Perform discharge planning by organizing discharge planning meetings as applicable, making referrals to home health agencies for follow-up care, ordering medical equipment as advised by the rehabilitation or nursing staff, sharing resources with resident/family for community supports (such as adult day programs, meals-on-wheels, emergency response units).
  • Notification to resident or family member of scheduled care plan meetings. Attending and facilitating care plan meetings or other family meetings as needs arise.
  • Provide individual support to residents and family members as needed.
  • Participate in chart audits to ensure quality of care is being given from a social services perspective and all admission and social work documentation is accurate and complete.
  • Participate in surveys/inspections made by authorized government agencies.
  • Investigate and work with interdisciplinary team in handling resident or family concerns or complaints. Maintain documentation of complaint in official log.
  • In collaboration with the Administrator, investigate allegations of abuse and neglect with utmost tact and confidentiality according to state and federal laws and regulations.
  • Honor all state and federal resident's rights.
  • Participate in interdisciplinary meetings as required. Includes, but is not limited to, At? Risk, QA, PDPM, Clinical Review, Stand-up/Stand-down, etc.
  • Participate in monthly Resident Council meetings if requested by the Resident Council or administrator.
  • Complete social services portion of all resident MDS assessments on time.
  • Work with the administrator to develop plans of correction for social service related deficiencies that are noted during surveys.
  • Interview residents using various assessment tools including Mini Mental Status Exam, Geriatric Depression Scale, Cornell Scale for Depression and others as needed.
  • Participate in Manager-on-Duty rotation.

Requirements of a Director of Social Services:

  • Master's Degree and Maryland State Licensure as a Licensed Certified Social Worker (LCSW).
  • Experience in health related facility is recommended.
  • Knowledge of nursing practices and procedures, as well as laws, regulations and guidelines that pertain to long term care, or must be willing to acquire.
  • Must possess leadership skills and be detail-oriented.
  • Good communication and interpersonal skills essential.
  • Computer literacy using Windows OS, Microsoft Office Suite, Google Suite. Knowledge of Point Click Care or other EHR.
