Job Title: Physical Therapist

Reports to (Title): Director of Rehabilitation

Supervises: (Positions) Physical Therapy Assistant, Technicians, Support Staff and Students

I.Position Summary:

Develops individualized treatment plan based on evaluation of patient. Provides direct patient care and delegates appropriately to support staff. Provides ongoing direction to patient care and evaluates outcomes. Responsible to actively participate in quality assurance/performance improvement activities to ensure processes are meeting the intended goal/outcome, to identify opportunities for improvement, and to be accountable for the quality of care provided in their respective area.

II.Position Requirements:

A.Licensure/Certification/Registration: RPT, state license or eligibility

B.Education: BA, BS or MS in Physical Therapy

C.Experience: One (1) year previous related experience

D.Special qualifications: CPR certified with American Heart Association

III.Degree of Supervision Required: Involves general guidance and direction by Clinical Supervisor and/or Department Manager.The employee will be expected to perform most ob duties independently and in accordance with established departmental andhospital policies and procedures.

IV. Ages of Patients Served:

This position requires competence in assessment, treatment, and/or care for the age groups indicated. The staff member must be able to demonstrate the knowledge and skills necessary to provide care, based on physical, educational, safety, and related criteria, appropriate to the age of the patients served in his/her assigned service area. The skills and knowledge needed to provide such care may be gained through education, training, or experience.

VI. Physical Requirements:

Must be able to climb, kneel, stoop and perform assigned duties under semi-stressful situations. Physical demands require performing heavy cleaning projects with short notice. Visual acuity is required in order to ensure that work is being done correctly. Work is moderately heavy with frequent lifting and carrying of objects weighing up to 25 pounds or more with assistance, and pushing up to 300 pounds. Work also involves considerable walking, standing, pushing and pulling of equipment. Frequent lifting of patients with assistance. Must have full use of upper and lower extremities and the ability to walk up to three (3) miles per shift. Must have valid driver’s license.

VII. Personal Protective Equipment:

This position requires the use of personal protective equipment during various tasks/activities. Personal protective equipment includes, but is not limited to, gloves, masks and safety glasses.
