GENERAL JOB DUTIES

Provide and monitor services to ensure that individual desired outcomes are being met and services are following State and federal Public Health/Aid and DHS guidelines.


ESSENTIAL JOB DUTIES

  1. Develop and monitor Services for individuals assigned.
  2. Monitor individual's status in relationship to their service plans monthly and document.
  3. Provide staff training in relationship to client's Service Plans.
  4. Facilitate and complete monthly house meetings.
  5. Initiated and coordinate the interdisciplinary process.
  6. Create implementation strategies based on the person-centered plan.
  7. Submit program evaluation and quality assurance data as required.
  8. Assure annual review of the client's facility contract and rights with individuals on assigned caseload.
  9. Provide emergency intervention/assistance for medical/behavioral client concerns.
  10. Provide individual or group counseling.
  11. Promotes and supports healthy lifestyles through diet, exercise and healthy choices.
  12. Develop and implement behavioral programming techniques.
  13. Adhere to all agency policies and procedures.
  14. Complete required 12 hours of continuing education hours annually.

ADDITIONAL JOB DUTIES

  1. Attend required in-service training sessions to maintain and/or upgrade skills.
  2. Attend home team meetings and weekly department meetings working as a team member with other agency staff.
  3. Teach specialized programs/classes and record client response and progress.
  4. Teach Relevant employee training classes.
  5. Travel routinely within the state.
  6. Serve as AOD on a rotating basis; assigned to rotation by supervisor.
  7. Remain current on required trainings and participate in other trainings as assigned.
  8. Other duties as assigned.

QUALIFICATIONS

  1. Bachelor's degree in a human service or related field, meet state licensing requirements.
  2. One year experience working with people who are developmentally disabled - required
  3. Effective communication skills: Listening, verbal and written.
  4. Valid Driver's License, driving record acceptable to Agency insurer.
