Job Summary:
The Family Service Coordinator for Early Intervention is responsible for providing comprehensive support and assistance to families with young children who are in need of early intervention services. This position plays a critical role in coordinating and connecting families with necessary resources and services to promote the well-being and development of their children. The Family Service Coordinator acts as a liaison between families, early intervention providers, and community resources to ensure a holistic approach to support.

Responsibilities:

  • Collaborate with families to assess their needs and develop an individualized family service plan for early intervention.
  • Coordinate and provide referrals to early intervention services, such as developmental assessments, therapy sessions, and educational programs.
  • Advocate for families to access appropriate early intervention services and ensure their rights are respected.
  • Conduct regular follow-up with families to monitor their child's progress and provide ongoing support and guidance.
  • Maintain accurate and confidential documentation of all interactions with families and services provided.
  • Collaborate with early intervention providers, healthcare professionals, and other stakeholders to ensure coordinated and effective supports for families.
  • Stay informed about early intervention programs, resources, and policies that impact families and utilize this knowledge to provide the most up-to-date information and support.
  • Participate in team meetings and professional development opportunities to enhance skills and knowledge specific to early intervention.


Qualifications:

  • Must be at least 21 years of age
  • Must have a valid drivers license and good driving record that meets insurance provider standards.
  • Must be able to pass all required backgrounds.
  • Bachelor's degree in social work, psychology, child development, or a related field preferred
  • Proven experience working with diverse families and children in early intervention settings.
  • Solid understanding of early intervention systems, services, and community resources.
  • Excellent interpersonal and communication skills, with the ability to build rapport and trust with families.
  • Strong organizational and time management abilities to effectively prioritize and manage multiple tasks.
  • Ability to work independently and collaboratively in a team-based environment.
  • Compassionate and empathetic approach in working with families and young children facing developmental challenges.
  • Proficient computer skills and experience working with databases or case management systems.
  • Knowledge of relevant laws, regulations, and ethical guidelines in early intervention and social services.
