Private Duty Nurse (LPN/RN) Needed for 1:1 Home Care

We have an exciting opportunity for a nurse to make a HUGE impact in a young man's life. We are looking for a nurse that is ready to take a step back from the chaotic hospital environment and work one-on-one with a child in their home. Home Health nurses are heroes in Centria's mind. They afford these children and their families a piece of normalcy in their lives. Without home health nurses, these children would live at the hospital never knowing what it feels like to be home. YOU can make a difference- even if it is only one day a week.

This young man Lives with his loving parents, 2 siblings, 2 dogs and a cat in a smoke free home in St. Louis Michigan. He is scheduled to tentatively discharge home in Late April/Early May. His care requires tracheostomy care, ventilator management and G-Tube feeds. The family is in search of a nurse that is willing to work nights to afford them the rest they need to properly care for him during the day.

Centria Healthcare is accustomed to employing nurses with little to no experience with trach/vent care experience. We are prepared to provide training to ensure our nurses and families are comfortable prior to having our nurse work solo. In fact, we pay for the training so there is no downside!!

​Responsibilities:

• Perform duties outlined in patient's plan of care
• TBI + SCI rehabilitation
• Pediatrics
• Ventilator and Tracheotomy care (Training available)
• Gastrostomy tube care
• Dispensing medications
• Observe client’s physical, mental and emotional conditions.
• Report any changes
• Document observed changes
• Communicates effectively with Clinical team to increase optimal outcomes
• Maintains professional nature within the home setting

​Qualifications:

• Excellent oral and written communication skills
• Ability to follow verbal and written instructions
• Ability to utilize computers/tablets and computer software
• Current state license as an RN or LPN
• Valid driver’s license and reliable transportation
• Current negative TB skin test or negative chest X-ray
• Current flu vaccine
• CPR certified
