Specialty: Physical Therapist

Job Description:

Performs Physical Therapy treatment procedures or delegates procedures to a licensed Physical Therapy Assistant or therapy aide based on initial and on-going client evaluation.

Evaluates Clients by utilizing appropriate diagnostic testing of muscle function and activities of daily living.

Develops and implements the client's plan of care in collaboration with the health care provider, other consulting health care professionals, family/care giver according to agency procedures and state and federal regulations.

Designs and applies an ongoing restorative program utilizing appropriate support personnel, i.e. family members, nursing staff, restorative aides, and Rehabilitation Aides.

Consults with the attending health care provider and obtains an order (when needed) for changes in the client's plan of care.

Ensures that assigned client's clinical records are complete, timely, and reflect coordination of care.

Prepares and uses necessary equipment.

Inventories and requisitions adaptive equipment and all other supplies needed.

Designs and constructs special equipment for individual and makes suggestions regarding adaptations of individual's work-living environment.

Assists in evaluating patients present equipment and offer suggestions or recommendations for changes to enhance the patients overall functional abilities.

Assures the safety and efficacy of equipment used to provide rehab services.

Collaborates with clients, families, healthcare staff and support services to achieve client goals and meet service expectations.

Provides client care in a service environment that demonstrates honesty, integrity, compassion, and trust.

Assures delivery of optimum service by reporting to work on time as scheduled, and/or providing appropriate advance notification.

Reports significant changes in the client's condition to the health care provider.

Qualifications:

Bachelor's Degree or equivalent - REQUIRED

At least 1 year of PT experience - REQUIRED

BLS, CPR certification

Active state license

Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.
