Outpatient only Schedule: Monday - Friday, 8am - 5pm 50% Adults/50% Kids EMR: Epic Allowed holidays: New Year's Day, Memorial Day, 4th of July, Labor Day, Thanksgiving Day, & Christmas Day Requirements CA License Active DEA before start Clean history BLS Immunizations: Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). Primary series plus booster, regardless of the date received or 1 dose of the current COVID vaccine (2023/2024 formulation) Chart closure within 72 hours 30-day notice for ending all assignments
