*We are accepting LCSW, LCPC & LMFT*

Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. We have been serving residents of Edgar County since 1968 though community education, emergency services, and outpatient care. Position Summary:

The position is responsible for completion of psychosocial/social service assessments and maintaining records in a timely fashion. Must exercise good working knowledge of other departments and community agencies to meet the patient’s needs. Is able to work in collaboration with other departments for the best interest of the patient. Must maintain organization and be flexible. Must demonstrate excellent verbal and written communication skills. Must be able to remain calm and approachable during crisis situations.

Essential Functions (Responsibilities/Accountabilities):

    • Diagnose, monitor, and treat mental health disorders
    • Demonstrate some knowledge of medical terminology.
    • Facilitate sessions with individuals, groups, and/or families
    • Collaborate with physicians, nurse practitioners and other providers regarding a patient’s care.
    • Provide patients with recommendations and provide referrals to other departments or community agencies as needed.
    • Complete assessments and documentation in a timely manner.
    • Create treatment plans based on patient needs
    • Advocate for patient’s human rights to be acknowledged in all decision-making.
    • Ability to respond to crisis situations

Position Requirements:

  • Licensed Clinical Social Worker
  • Master’s degree
