Position Summary: The Cultural Liaison performs outreach, care coordination, translation, and interpretation for Latinx people to improve access to Behavioral Healthcare.

 

Our ideal candidate for our Cultural Liaison position will have their BA or a combination of life experience and professional experience to fulfill the position's responsibilities.  They will also have:

  • 1-3 years of experience delivering case management services.
  • 1-3 years of experience working with the Latinx population.
  • Knowledge of mental illness and substance use issues.
  • Knowledge of community health and mental health resources.
  • Preferred Spanish speaking.

Salary:   $24.63 (Base Rate $22.13 + Locale $2.50) - $25.96, hourly and considers preferred experience.

Physical demands: Employee must occasionally lift and/or move 15 pounds up to 50 pounds on an occasional basis. Employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between AHS locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. AHS may make reasonable accommodations to enable individuals with disabilities to perform the essential functions.

  

Essential Functions:

  • Participates as a member of the behavioral health team to enhance the care for Latinx clients in accordance with policies, rules, regulations, and directives.
  • Utilizes a variety of engagement approaches and care coordination to work with Latinx client populations to improve access to care and patient experience of care.
  • Conducts outreach, education and service coordination for Latinx patients.

  

Additional eligibility requirements:  Annual Flu immunization,  Annual TB screening, CPI training within 90 days of hire, BLS PRO certification required within 90 days of hire.

Specific services include the following:

Mental Health Outpatient (MHOP) services: AHS’ MHOP services include individual, family, and group therapy, psychiatric assessment and care, and wellness and healthy living education and support. Care is delivered in teams that may include mental health clinicians, psychiatric providers, case managers, vocational specialists, peer specialists, and wellness advocates.

Crisis services, respite, and withdrawal management: AHS provides 24/7/365 crisis care, as well as social withdrawal services, acute psychiatric care, and mobile crisis response.

Co-responder: AHS collaborates with law enforcement and municipalities in Montezuma County as well as the City of Durango to provide clinical support when calls involve someone experiencing behavioral health challenges.

Substance Use Disorder treatment: AHS also offers comprehensive treatment (including several evidenced-based practices) for individuals with substance use disorders, including individual therapy, group therapy, an intensive outpatient program, and education classes. AHS collaborates with local law enforcement programs and participates in the regional Drug Courts to help residents address substance use issues that have resulted in involvement with the legal system.

Integrated Healthcare: AHS provides whole-person healthcare with a multi-disciplinary approach that focuses on wellness outcomes for all patients. This includes primary care, pharmacy, and dental services, in addition to behavioral health services.
