THE VITAL ROLE YOU HAVE

As a School Counselor - Care Manager, you'll play a vital role in providing care management to adults and children in our community. You'll focus on the strengths of individuals and families, assisting them in gaining access to needed medical, social, educational, and supportive services. Your responsibilities include managing person-centered care plans, overseeing treatment adherence, providing crisis support, and establishing relationships with treatment settings. Additionally, you'll provide clinical services, including individual therapy, group therapy, and play therapy, in collaboration with teachers and educational assistants in the school setting.

QUALIFICATIONS

• Master's degree in behavioral science (social work, counseling, or psychology)
• Excellent communication and networking skills

Experience in group, family, and individual therapy is preferred.
