Accessible, Credible individuals to join a great team. Responsibilities:
- Provides telephonic outreach and in-person consultation to members identified as high risk
- Assist in identifying gaps in care utilizing NextGen
- Collaborate with healthcare team to optimize patient adherence to medical plan of care
- Proactively outreach to members newly diagnosed with a targeted chronic condition
- Educates patient and family members about preventative screenings while identifying barriers to care
- Provides patient resources
- Interacts with healthcare team
Qualifications:
- Completion of a Medical Assisting or LPN Program is required
- MA Certification or Registration or Ohio LPN License is required
- Primary care experience preferred
- Experience with quality reporting is preferred
