Homeless Navigator/Targeted Case Manager

The Homeless Navigator/Targeted Case Manager (HN/TCM) will work to secure housing for clients experiencing homelessness. Will work with clients before and after successful housing by assisting with any applicable needs they may have. Will work with individuals and households to improve quality of life, helping clients improve their weaknesses and build on their strengths, while treating them with dignity and respect. Responsible for obtaining, reporting, and maintaining required documentation and performing administrative tasks in a thorough fashion. Must be a “team player” with a high level of focus and drive and an ability to work independently.

Must have completed a bachelor's level of education in a mental health or related field to be considered for this position.
