POSITION SUMMARY

As a member of the care team, the Social Worker develops care plans to address patient’s social determinants of health, behavioral health high risk tracking, and assists patients with community resources to help with financial, social, spiritual, mental, and physical needs all in a holistic manner.


KEY RESPONSIBILITIES

  • Establishes rapport and working relationship with patients in order to develop care plans for individual patients to best meet patient’s needs.
  • Interviews patients to assess for specific needs including social determinants of health, community resources, and arranges for appropriate interventions.
  • Addresses barriers to care such as access to transportation, low literacy, and insurance barriers.
  • Works as Case Manager with high risk suicidal and MAT patients. Provides Case Management and complete Care Plans in the electronic medical record.
  • Works as part of a CCHS interdisciplinary team. Serves as liaison for home health, nursing home placement, and coordinates referrals to behavioral health services and substance abuse programs.
  • Collaborates with various outside agencies to assist patients in receiving available services.
  • Attends weekly supervisory meetings with the Director of social Work; and required staff meetings.
  • Is a mandated reporter for suspicion of abuse /neglect of children, adults and the elderly.
  • Maintains accurate and concise documentation in the patient electronic record daily.
  • Adheres to the Social Work Code of Ethics, as stated by the National Association of Social Workers.
  • Performs other duties as assigned.


Requirements


POSITION REQUIREMENTS

Education: Bachelors degree in Social Work (working in the healthcare field in similar fashion for no less than two years will be considered in lieu of graduation from an accredited University with a Bachelor's Degree. Must be on track to complete a Bachelor's Degree in Social Work within an agreed upon time frame).

Experience: 1-to-2 years working with patients in a healthcare setting; working knowledge of basic medical terms

Licenses or Certifications: LBSW Preferred
