Supports HH and Hospice clinical excellence by focusing on onboarding, training, regulatory, compliance, client experience, skills, competency training, and clinical standards development built on a foundation of industry evidenced-based practice.

Responsible for assisting with research, data analysis, role-based improvement efforts, and functioning as a subject matter expert for key service providers such as the multidisciplinary team. Partners across the Home Health and hospice practice to facilitate proper development, deployment, and monitoring of standards for service excellence initiatives.

MINIMUM QUALIFICATIONS:

  • Exemplifies characteristics of our mission including compassion, excellence, and reliability.
  • Holds a current license in good standing as a Registered Nurse in the state of practice.
  • BSN Preferred.
  • Master’s Degree Preferred, MSN, MBA, or MHA
  • Must be willing to obtain OASIS certification within one year of employment.
  • Seven to ten years of Home Health/hospice leadership experience
  • Five years’ experience in developing and delivering education as an educator, clinical director, or administrator. Experience with professional development programs utilizing multiple modalities and delivery systems including educational courses, distance learning, and online LMS educational platforms.
  • Multisite/State Experience Preferred
  • Experience and knowledge of Home Health and Hospice accreditation requirements
  • Demonstrated subject matter knowledge and expertise.
  • Demonstrated record of strong interpersonal skills.
  • Competence in basic PC skills required to perform job functions.
  • Ability to read, write, and effectively communicate in English.

PRIMARY RESPONSIBILITIES:

  • Assess and evaluate the needs of the clinical operations/employees according to state and federal regulations, current nursing standards, accreditation standards, and policies.
  • Is the home health and hospice clinical subject matter expert in terms of operations, training, field compliance, and field process standards for survey/accreditation efforts.
  • Conducts research and evaluates the “current state” for a body of work to improve hospice clinical experience.
  • Supports the Director of Quality/RCD with recommendations for clinical operations and performance improvement for individual offices based on key performance indicators and educational needs.
  • Works in collaboration with Clinical Managers, Regional Directors, and other key stakeholders to enhance orientation and onboarding for new employees.
  • Develop and maintain a working knowledge of relevant regulations, laws, accreditation standards, and agency policies and assist in interpretation of and system-wide compliance with standards.
  • Monitors upcoming regulatory changes and supports the creation of education, processes, and standards to maintain compliance.
  • Assists in development of compliance and clinical practice standards.
  • Identifies practice gaps for educational programs (face-to-face and online), and applies data gathered to develop content with target learning outcomes that can be measured throughout all divisions and by distinct roles.
  • Identifies stakeholders to include in planning sessions for new or focused clinical projects, that may include designing, delivering, and evaluating the effectiveness of new programs and future training materials.
  • Collaborates with various subject matter content experts to integrate core competencies, evidence-based curriculum, case studies, and other resources into didactic and self-paced learning programs.
  • Provides support to clinical operations with a focus on delivering quality patient care through customer centric education programs that focus on standards, regulations, and evidenced-based practice and are efficient and cost-effective.
  • Manages the content of educational programs, and online learning courses, as well as facilitates the delivery of new employee onboarding.
  • Partners with the Chief Clinical Officer and Regional Clinical Directors to expand the formal mentorship/preceptor program to enhance new employee onboarding.
  • Develops and integrates key educational priorities inclusive of policy, quality, and regulatory standards to enhance patient and office outcomes and to enhance the learning experience. Prioritizes educational projects to closely align and meet patient, organization, and employee needs.
  • Cultivates relationships and utilizes forward thinking to deliver, promote and solicit buy-in for meaningful clinical opportunities in a variety of formats to employees and leaders.
  • Develops and builds strong relationships with employees by focusing on timely responses to requests and using a service excellence approach to follow-through and duties.
  • Participates in the evaluation of clinical programs by identifying and monitoring data and trends; implements changes to improve learning outcomes.
