An Excellent Opportunity

Mountain Valley is currently seeking a PRN Hospice Social Worker to work in our Surry County service area. The Social Worker helps to meet the needs of individuals and families affected by life-limiting illness and end of life issues. Working with populations of varying cultures, ages, socioeconomic classes and nontraditional families, the social worker must be prepared to assess needs and intervene appropriately.


Duties

  • Assess patients and families and develop care plans to enhance family coping skills with terminal illness.

  • Reassess and revise care plans in response to newly identified needs and altered goals of care.

  • Facilitate communication between patient/family/team members.

  • Educate patient/family about anticipatory grief, dying process, grief and loss.

  • Make referrals as needed to volunteer coordinator, chaplain, or community resources.

  • Collaborate with interdisciplinary team members and advocate for patient needs.

  • Maintain appropriate documentation in a timely manner.

Qualifications

  • Graduate from an accredited school with bachelor’s or master’s degree in social work, sociology, psychology or rehabilitation counseling.

  • Minimum of one year social work experience in a healthcare setting required.
    The willingness and ability to travel to patients homes, SNFs ALFs, hospitals, etc.
