Summary: Provides primary wound care; assessing, treating, documenting, and tracking all wounds and skin treatments; responsible for assessing nail care treatment and requirements and providing routine care.

Required Knowledge, Skills and Abilities:

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports. Ability to work as a team member with the nurses and nurse aides of Birmingham Green. Understanding of the behavior and needs of elderly people. Ability to communicate effectively to the residents, families, visitors and staff. Accuracy in documentation is essential.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several variables. Excellent organizational skills and accuracy in following directions, observing, interpreting, and recording essential information.
  • Current RN License in the State of Virginia, CPR, IV Therapy Certificate.

Education and Experience:

  • Graduation from a state approved nursing program.
  • Current license to work as a Registered Nurse in the Commonwealth of Virginia.
  • One year experience; understanding of the geriatric resident and their needs.
  • Excellent communication, interpersonal and documentation skills.
  • WOCN certification desired but not required.

Essential Functions:

Wound Care:

  • Conduct and document a thorough assessment of each resident’s skin upon admission and throughout the resident’s course of treatment.
  • Under the supervision of DON, assist the implementation of the resident’s treatment plan.
  • Comply with evaluation, treatment and documentation guidelines of Birmingham Green wound protocols.
  • Complete required documentation in an accurate and timely manner.
  • Provide regular resident skin status updates to appropriate personnel.
  • Attend and participate in facility wound rounds bi-weekly or as required; attend and participate in department/facility meetings as required.
  • Maintain ongoing communication with physicians concerning skin treatments.

Clinical Nail Care

  • Responsible for assessing Nursing Home resident’s nail care/treatment requirements.
  • Responsible for trimming all routine toe nails as needed. Will communicate needs of residents who require routine fingernail care to Head Nurses and CNAs. Will be responsible for follow up.
  • Responsible for coordinating Podiatrist treatment of residents who are insulin dependent or receiving Coumadin (or other blood thinner) and assisting Podiatrist with care/treatment as needed. Responsible for keeping a log of such treatment.
  • Responsible for documenting routine nail care given in resident’s charts.
  • Responsible for cleaning and maintaining nail care instruments and adhering to sanitary cleaning procedures.
Related Duties and Responsibilities:
  • Facilitate problem solving and open communications with nursing staff. Act as a resource person for nursing personnel and other staff.

  • Act in compliance with Birmingham Green’s regulatory and professional standards and guidelines.
  • Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical currency.
  • Adhere to facility policies and procedures and participate in facility quality improvement and safety programs.
  • Demonstrates ability to carry out procedures following established standards of care.
  • Maintains all certifications necessary for employment.
  • Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary.
  • Commits to continually improving his/her skills through participations in education opportunities whether offered on the job, within, or outside of the facility, to increase knowledge of work-related issues.
  • Maintains a current knowledge of federal, state and other regulations applicable to job.
  • Other duties as assigned.
