What is PACE North? PACE stands for Program of All Inclusive Care for the Elderly. We are a new and unique option for individuals 55 and older who qualify for nursing facility level of care and wish to remain in their homes as they age. What does that mean? We care for the elderly in daily both our state-of-the-art day center on Garfield near Hammond during regular business hours and as needed in their home. Never heard of us before? That’s okay, we’re new to the region. We began serving the elderly in 2019 and our service model is so popular it’s growing quickly!

What does a Social Worker at PACE North do? You can expect to be responsible for direct social work case management services for our participants, including participant screening, case management, referrals, and counseling. Social Workers are a decision-making partner in our Interdisciplinary Team and are heavily involved with care planning, assessments, and re-assessments. Social Workers also travel to our participants’ homes for site visits when needed, participate in our quality planning process, and have a hands-on opportunity to directly impact quality of care. Best of all, PACE North has most of the resources our participants need within our organization and they’re easily accessible!

Requirements:

What qualifications should you bring to the table? We’re looking for that special person who loves serving others with a positive approach. We’d appreciate at least 1 experience in an elderly patient setting, either a bachelor’s or master’s degree from an accredited School of Social Work with a valid social work license, CPR certification (we can train CPR if needed!), valid driver’s license, and be medically cleared for communicable diseases. With the driver’s license we also ask for valid auto insurance and reliable transportation.
