General Description: The Collaborative Care Specialist serves as a Social Worker for the Network and strives to achieve functional restoration to medical patients being seen by their Primary Care Physician and or their team. Additionally, the Collaborative Care Specialist will case manage/care coordinate individuals who may receive these integrated services.

Duties:

· Provide counseling to individuals, couples and groups referred by Network providers on a timely basis.

· Provide support, educate, and counsel patients on reaching goals associated within the primary care setting.

· Represents the Network in the social work profession by assuming an active role in local, state or regional societies.

· Provide assessment, short term treatment, and diagnosis to the medical population who have co-morbid medical and behavioral health needs identified by the PCP and or their team.

· Provide intervention services including but not limited to consultation, referrals, tracking, monitoring, and liaison with other medical professionals including but not limited to Psychiatry.

· Ensure effective and efficient communication between all providers in a patient’s care team.

Qualifications:

· Previous work history as Therapist, Licensed Clinical Social Worker, Licensed Professional Counselor, or Licensed Marriage and Family Therapist is required.

· Current licensure in the Commonwealth of Pennsylvania and/or State of Ohio (depending on location of practice site).

· Minimum of two (2) years experience after graduation.
