Summary:

The Occupational Therapist provides prescribed treatments concerned with improving or restoring patient's ability to function independently.


Essential Job Function of the Occupational Therapist:

Evaluates and identifies the problem and the patient’s level of functioning by administering diagnostic and prognostic tests as directed by the physician and in accordance with Agency policies including OASIS assessments at appropriate time points. Designs appropriate plan of treatment for individual’s need and complete appropriate documentation. Ensures initial and ongoing assessment and evaluation of patient/family response to therapy. Evaluates the effectiveness and outcomes of care and identifies barriers to desired outcome.

Sets goals related to needs of the patient. Selects & teaches task-oriented therapeutic activities to restore "independent" functioning, as well as providing direct rehabilitative services under insurance and reimbursement guidelines and maintains records, via clinical and progress reports, for each visit. Plans, evaluates, implements, and teaches patients and family members to provide for continuous therapeutic activity programs during care and discharge. Instructs other Agency personnel, family members and significant others in methods of assisting patients according to a written plan of treatment.

Educates and consults with patients/families and with organizational staff. Prepares and maintains appropriate clinical and administrative records in a prompt and comprehensive manner, documenting patient limitations/interventions and progress. Timely records all assessment and evaluation data, treatments and patient's response to therapy intervention and addresses progress towards established goals. Completes paperwork within 24 hours of visit.

Assesses and accurately documents patient status related to OASIS information. Designs and fits orthotic and self-help devices. Evaluates equipment needs to increase functional and independence levels. Plans and implements vocational and pre-vocational assessment and training.

Develops and revises a plan of treatment and care as indicated in collaboration and consultation with the physician, ITC members, and other healthcare team to achieve goals that is not a duplication of active physical therapy programs. Actively participates in patient care conferences, in-services, staff meetings, rehab meetings, continuing education programs, Clinical Records Review, and other meetings as requested. Assesses, records and reports patient’s status and progress to the physician and health care team. Provides regular supervision to the COTA and home health aides as appropriate via weekly meetings and phone communication, and directs the COTA in carrying out the plan of treatment as established by the therapist.


Requirements:

Graduate of an approved Occupational Therapy curriculum.
One-year of clinical experience preferred.
Occupational Therapist currently licensed to practice occupational therapy in North Carolina.
Maintain current CPR certification (BCLS).
