The Village Project Inc. - Nymbani Program Case Manager

Education: Bachelor's degree in Social Work or a related field

Experience: Minimum of two years of experience in case management or a closely related field.

Supervisor:  Reports to a designated supervisor assigned by the Executive Director

Responsibilities:  As a Case Manager your primary responsibilities include supporting and advocating for clients who have been negatively impacted by the War on Drugs.

  1. Recruitment of Clients:

    • Your role begins by identifying and reaching out to individuals affected by drug-related issues. This involves actively seeking potential clients who may benefit from the services offered by the Nymbani Program. 
  2. Case Management:

    • As a case manager, you’ll work closely with each client and their family members. Your goal is to provide comprehensive support, address their unique needs, and guide them through the recovery process.
    • You’ll conduct intake interviews, gathering essential information about the client’s background, challenges, and goals.
    • Assessing Clients’ Needs: You’ll evaluate their specific needs, considering factors such as mental health, substance dependency, employment, and overall well-being.
    • Referrals: Based on your assessment, you’ll make referrals to mental health clinicians and connect clients with external resources.
    • System Navigation: You’ll assist clients in obtaining essential documents, such as driver’s licenses or California Identification cards. This helps them reintegrate into society and access necessary services.
    • Transportation Support: Occasionally, you’ll provide transportation assistance for court hearings, job interviews, and equine therapy appointments.
    • Program Performance Reports: Regularly submit quarterly reports detailing program outcomes and progress.
    • Outreach and Marketing: Develop and execute outreach activities to raise awareness about the program. Launch marketing campaigns to promote its services.
    • Online Presence: Maintain an active social media presence to engage with the community and share program updates.
    • Collaboration and Networking: Attend both weekly and monthly internal meetings. Additionally, participate in meetings with community leaders, civic organizations, elected officials, and other stakeholders.
    • Adaptability: Be prepared for other duties assigned by your program supervisor or executive director.

Core Skills:

  • Knowledge of the Criminal Justice System: Understanding of the processes and challenges faced by individuals reentering society post-incarceration.
  • Crisis Intervention: Expertise in managing urgent situations with calmness and efficiency.
  • Advocacy: Strong advocacy skills to navigate resources and support for clients, including legal aid, housing, and employment opportunities.
  • Program Development: Experience in developing and implementing reentry programs tailored to the needs of individuals affected by the war on drugs.
  • Communication: Excellent verbal and written communication skills for interacting with clients,  clients family members, stakeholders, and community partners.
  • Cultural Competency: Sensitivity to and understanding of diverse populations, with a focus on reducing recidivism among marginalized groups.
  • Documentation: Proficient in maintaining accurate and confidential client records.
  • Networking: Ability to build relationships with community resources, non-profits, and government agencies.
  • Continuous Learning: Commitment to staying informed about best practices in social work and reentry strategies.
