Schedule: Full-time | 80 hours per pay period | Monday-Friday | 8 am – 4:30 pm
Scope:
- Responsible for consultation, direction, and program development of bereavement services in Hospice.
- Responsible for bereavement support to hospice families in coordination with the Interdisciplinary Group (IDG).
- Development and provision of community bereavement support and education related to end-of-life and grief and loss issues, to include Ethos contract facilities.
- Ensure quality and consistency to meet changing needs of patients, families, referral sources, communities and Hospice.
- Provide orientation to new staff on bereavement program.
- Provide education to staff on topics related to grief and loss/death and dying.
Key Responsibilities:
- Provide coordination and direction for the current bereavement services, community education and support services related to end-of-life care.
- Following a patient death, provide initial bereavement needs and psychosocial assessments of clients desiring bereavement services.
- Develop bereavement plan of care with IDG and client that identify the needs, outcomes, and interventions for each client.
- Document all client contact and interventions in a timely and accurate manner.
- Provide ongoing bereavement services and availability for 13 months from patient date of death.
- Refer clients to support groups and community resources as appropriate.
- Responsible for the development, delivery, and evaluation of bereavement services and community education.
- Collaborate with Quality Manager, Life Enrichment Manager & Director in the development, delivery and evaluation of bereavement services.
- Collaborate with Marketing and Sales Team in the promotion and marketing of bereavement programs, community education and support services.
- Develop and maintain policies and procedures related to the delivery of hospice bereavement services to comply with regulatory requirements.
- Engage in opportunities for continuing education in areas of bereavement, grief and loss, and death and dying.

Education and Experience Required:
- Bachelor’s degree in social work.
- Professional knowledge and experience with emotional, physical, psychosocial, cultural, and spiritual effects of death and dying/grief and loss.
- Professional training and experience with grief and loss counseling of individuals, families and group facilitation. Preferred, but not required.
- Training and experience in family dynamics and psychosocial assessment.
- Strong communication skills.
- Experience in public speaking.
- Must complete and pass a state required background study.
- Able to physically perform the duties required by the position.
- Required to have the ability to read, write and speak English well enough to communicate with staff, patients, families and facilities.
