Our Mission:

St. Ambrose Housing Aid Center provides comprehensive housing services and quality affordable homes to meet the challenges facing homeowners, renters and neighborhoods in the Baltimore Area. As Baltimore’s oldest non-profit housing provider, St. Ambrose has served more than 125,000 families since our inception in 1968. Our innovative programs help over 3,000 low- and moderate-income families annually.

Overview:

Through positive relationship building and managing transitions, the Youth Case Manager helps youth identify and achieve goals related to maintaining safe, stable housing, employment, education, and increasing life skills. In coordination with the Host Home Coordinator, the Youth Case Manager will develop professional relationships and partnerships with other community service providers and provide community engagement or program outreach as required.

Key Responsibilities:

  • Conducts initial assessment and evaluation for program adaptability and the formulation of an Individual Housing Services Plan (IHSP)
  • Provide ongoing supportive case management services and follow-up services to clients providing youth flexible, responsive case management that address their variety of needs
  • Conducts weekly in-home follow ups, monthly progress updates, and reviews IHSP & updates goals on an ongoing basis
  • Conducts, reports, and monitors inspections at both the Host’s homes and HOPE house

Key Qualifications:

  • Bachelor Degree in Social Work or related field
  • Three years of related experience in case management, community engagement, and/or community-based or family systems therapy
  • Strong communication, organization, and counseling skills
  • Understanding and empathy for the needs and concerns of youth served

Physical Requirements:

This position normally requires the physical demands of standing, walking, bending, lifting or performing other work requiring medium exertion, talking, and hearing on a regular basis to perform the job requirement. These physical demands are required up to 85% of the time.
