Hannah's Children's Homes is seeking an FFA Social Worker to conduct home study assessments and/or carry caseload for our agency service areas as follows:

  • Social Worker agrees to represent the Agency in a professional manner at all times with families and collateral contacts.
  • Assignments will be offered based on Social Worker's availability and quality of work. Identifying case information will be provided once initial acceptance of the case has been confirmed.
  • Social Worker will carry a small caseload of foster children in care and provide assessment needs and service plans and complete visitations and quarterly reports.  Work alongside Resource Parent to ensure care including medical, dental, school, visitation with biological family, therapy, etc. 
  • Social Worker will contact the appropriate point of contacts with clinical, procedural, & regulation requirement questions; when any person in the home has a criminal history, TB or other contagious disease, terminal illness, or history of mental illness or psychiatric treatment.
  • You will be encouraged to contact the other admins in the program directly for any issues or concerns that may arise.
  • Three home study interviews, the Home Study Report, Questionnaire II, SOC 817 Checklist of Health & Safety Standards, Psychosocial Inventory, Case Notes regarding all client and collateral contacts, and any other pertinent documents will be submitted for review within 90 calendar days of the initial referral.
  • The completed SAFE Home Study will be sent as an attachment in an e-mail to the Director of Adoptions for review. Signed original signature pages, approval page and psychosocial assessment page, will be mailed to the Agency, along with the original signed Questionnaire II, SOC 817, and Psychosocial Inventory as part of the home study packet.
  • Any changes or corrections to the report will be requested by the agency within 30 days of Hannah's receipt of your final draft.
  • Confidentiality of case information 11111st be maintained at all times! Any confidential information obtained during the course of your assignment must be returned to the agency office or permanently destroyed upon final approval of the assignment.

  • Any change in your availability, change in your contact information, or any circumstance that might affect the responsible completion of an assignment, will be reported to agency adoption staff promptly so that the pending assignment may be handled efficiently.

  • Other conditions and/or assignments may apply as agreed upon by the Social Worker and the Director of Adoption Services and/or Foster Care Social Work Supervisor.

Qualifications:

  • Prior experience with Home Studies or able to obtain Home Study certification
  • Master's degree or higher from an accredited or state approved graduate school in social work or social welfare, marriage, family, and child counseling, child psychology, child development, counseling psychology or social psychology
  • Bilingual ability in Spanish preferred but not required
  • Word Processing; General Office Skills
  • Excellent Reading and Writing Skills
  • Must pass pre-employment physical exam, TB and drug screening
  • Ability to commute daily and continuously to various sites for prolonged periods of time
  • Ability to lift 10 pounds for filing
