JOB SUMMARY:
- Responsible for planning, managing, implementing, and evaluating the operations of two Case Management departments at RWJ Barnabas Health: Monmouth Medical Center - South Campus and RWJ Hamilton Hospital,
- Includes oversight of effective resources utilization and the provision of high quality and excellent care,
- Work in collaboration with administration and physician leadership to establish measurable standards and outcomes,
- Oversee nurse case management and social work services,
- Ensure continued development of the program to improve organization performance to enhance provision of services
MINIMUM QUALIFICATIONS AND REQUIREMENTS:
- New Jersey Registered Nurse BSN OR Licensed Clinical Social Worker
- Masters Degree in Nursing, Social Work, Healthcare Management, or comparable field preferred
- For Social Work, LCSW required,
- Case Management Certification to be completed within 12 months of hire,
