About the Job

If you want to make a positive impact in the lives of young adults, then Waypoint is looking for you. Waypoint NH's Transitional Living Program (TLP) in the Manchester, NH area is seeking an enthusiastic and passionate care coordinator / case manager. The care coordinator works collaboratively with young adults (ages 18-22) who are experiencing homelessness and looking to the TLP as a safe, stable, temporary housing option. The Care Coordinator will provide information and outreach to young adults who may be eligible prior to program enrollment; intensive case management and independent living skills building with residents in the program, and follow-up aftercare supports to those who transition out of the program. The Care Coordinator will work in partnership with the live-in House Mentor and Housing Support Staff to provide support to the youth and the house. Partnership with facilities maintenance is also required. Must have strong communication skills and an ability to partner with multiple communities and state partners to assist young adults in obtaining stability, increasing self-sufficiency through education and employment, and transitioning to safe housing.

Requirements:

  • Bachelor’s Degree required in Human Services, or related field
  • A minimum of 3 years of similar experience is required, particularly as it relates to disconnected and vulnerable youth (lived experience is greatly helpful)
  • Working knowledge of substance use, behavioral health, and knowledge of community resources is a plus.
  • Must pass a criminal background check with no history of abuse, neglect or exploitation and a motor vehicle record check
  • Valid Driver's License, reliable transportation and auto insurance that meets the company's minimum liability requirements of $50,000/$100,000
  • Ability to communicate verbally and written in a professional manner
  • Ability to create positive relationships with clients and fellow employees
  • Self-motivated, energetic, enthusiastic, with a passion for working with young parents and their children.
