Program for Assertive Community Treatment (PACT) is an intensive, team-based behavioral health service that offers individualized support in the community to people with serious mental health conditions and high service needs. PACT assists individuals in their recovery from mental health conditions and in developing fulfilling lives, as well as increasing their time and engagement in community settings rather than institutions such as hospitals and jails.

What you'll do

  • Under general supervision and working within a team orientation, facilities the team's tasks of assessing, screening, counseling and supporting clients through the performance of clinical, case management and support services to enhance, meet or restore their capacity to function.
  • Facilitates the team development of treatment plans. Provides ongoing clinical support and training to team members to assure clients receive the best possible care.
  • Provides face-to-face individualized clinical support services for consumers who need consistent contact to ensure the highest level of optimal psychosocial functioning.
  • 24 hour on-call coverage on a rotating schedule
  • Consult with other service providers and resource people regarding the assessment, treatment and rehabilitation needs of the persons served
  • Coordinate case documentation for up to ten persons served
  • On a regular and continuous basis, exercise sound judgment and assume responsibility for decisions, consequences and results having an impact on people, costs and/or quality of service within the functional area.

What you'll bring to the team

  • A master's degree in behavioral health science or counseling specialty from an accredited institution.
  • Two years of post-bachelor's experience working with individuals with serious and persistent mental illness.
  • Must have and maintain a valid driver's license with an acceptable driving record
  • Must demonstrate initiative, creativity, be dependable, reliable and work well independently.
  • Must be able to exercise independent thinking and good judgment under all circumstances.
  • Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
  • Must be sensitive, flexible and responsive to gender, race, ethnicity, socio-economic status, religion, age, sexual orientation or any other special needs as reflected in the ability to communicate with staff and/or clients.
