One Vision is looking for a Full time Outcome Coordinator for our Mason City Team!

The Outcome Coordinator is responsible for coordination and integration of the Individual Support Plan (ISP) for up to 25 assigned persons served of One Vision (OV), monitoring the ISP and progress on goals/objectives and supports to ensure
persons served needs are met. Ensures people served are safe and their personal welfare and rights are
supported and respected. Ensures that services provided operate within federal, state, and national
accreditation, along with One Vision policies, procedures, practices, and guidelines.
Starting hourly wage $22.00 per hour. (Currently offering a $1500 sign-on bonus)
- . BA/BS degree in a human service-related field or RN license with 1 year of experience in supporting people with disabilities. Or 5 years of progressively responsible experience/equivalent education.
- Completion of OV Med Administration within 6 months of hire.
- Moderate Computer skills needed for documentation.
- Ability responds appropriately to behavior, crisis, and emergencies to utilizing Mandt procedures.
- Valid driver's license with proof of insurance.
- Requires some physical activity bending, kneeling, and lifting or use of lifts.
- Offer of employment contingent upon background check and drug screen.
