Care Manager (In Home)
SUMMARY:
The Care Manager is responsible for coordinating, implementing and supervising the delivery of services to agency clients in alignment with the company’s mission and standards of care. This position is accountable for adherence to regulatory and agency requirements for service provision and ensures compliance with and maintenance of client records. The Care Manager will rotate the On-Call phone with other administrative staff.
