SUMMARY

Provide individual and group addiction counseling, case management services and program activities for the clients of the program.

PRIMARY RESPONSIBILITIES

  • Engage all clients by understanding and addressing their needs within the scope of their recovery.
  • Provide Individual and group support to assigned clients and collaborate with their providers.
  • Establish a recovery plan with each individual.
  • Build relationships with other substance use providers in the area.
  • Coordinate substance use services and community based referrals including Fuel Assistance, Food stamps, food pantries, and other services.
  • Provide related information and referrals to community-based resources including detox, IOP, Medication Assisted Treatment (Methadone, Suboxone, Vivitrol).
  • Maintain accurate client files and records; complete required paperwork.
  • Drive clients to appropriate recovery activities.
  • Attend & participate in team meetings as requested and communicate effectively with clients and staff in other areas.
  • Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements.
  • Other duties as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS

  • Certification or license in substance abuse counseling (CADAC or LADAC) or at minimum enrolled in a certificate program.
  • Strong written and verbal communication skills.
  • Valid driver’s license, reliable transportation and ability to meet insurance standards.
  • Bilingual Spanish language skills.
  • Ability to work remotely at other appropriate program sites.
  • Ability to work in a multi-disciplinary team with medical providers, case managers, and mental health specialists.

ORGANIZATIONAL RELATIONSHIP

  • Directly reports to Open Door Social Service Program Director.
  • Direct reports of this position are none.

PHYSICAL REQUIREMENTS

  • Ability to drive
  • Ability to ascend and descend stairs.

WORKING CONDITIONS

As part of the responsibilities of this position, the Substance Use Program Counselor will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.
