DAY TO DAY AS A CASE MANAGER REGISTERED NURSE

As a Full Time Case Manager Registered Nurse at Beacon of Hope Hospice, you will play a vital role in supporting all nursing services and collaborating with interdisciplinary teams. You will adhere to the highest standards of clinical practice, ensuring that our patients receive the best possible care. Your expertise in EMR and paper documentation will be crucial in accurately documenting patient information and maintaining comprehensive records. Additionally, you will be responsible for monitoring compliance with performance reports, ensuring that all patient care needs are met and exceeded.

WHAT WE'RE LOOKING FOR IN A CASE MANAGER REGISTERED NURSE

To be successful as a Full Time Case Manager Registered Nurse at Beacon of Hope Hospice, there are several skills and qualifications you should possess. First and foremost, you must be a graduate of an accredited nursing school and hold a current license to practice nursing in the state of practice. Having at least one year of experience in general nursing practice is required, with preference given to candidates who have experience in hospice, palliative, or end-of-life care. Strong communication skills are essential in this role, as you will be working closely with patients, their families, and interdisciplinary teams. The ability to collaborate effectively and work well with others is crucial, as you will be part of a compassionate and caring team.

The role of a Case Manager RN can be demanding, so the ability to function well under pressure and independently is important to succeed in this job. As a Case Manager RN, it is vital to adhere to the standards outlined in the Nurse Practice Act and to consistently provide high-quality care to our patients. Knowledge and skills required for the position are:
