General job duties and types of procedures required:

The example diagnoses within the APCU scope of care serve as guidelines for appropriate patient placement, but are not limited to the following:

• Chronic or sub-acute respiratory failure requiring mechanical ventilation

• High oxygen requirements (e.g., FiO2 > 0.50 in order to maintain SaO2 greater than 90%

• Advanced pharmacological interventions

• Advanced Pulmonary Artery Hypertension requiring treatment with epoprostenol sodium (Flolan) or treprostinil

(Remodulin) by continuous infusion administration

• Frequent assessments or medical interventions that are not available on the specialty medical surgical telemetry units

• Intracranial hemorrhage whom require intracranial monitoring through a ventriculostomy

Assessment and Reassessment:

Assessment of the patient is initiated within 15 minutes of admission. The assessment is conducted according to the

Admission Data Documentation on the Adult/Pediatric Patient. Vital signs are taken at least every 4 hours. All collected data is interpreted by a registered nurse and appropriate interventions are implemented as identified and documented.

Frequency of reassessment is based on individual patient needs and is conducted at a minimum of every 6 hours.

Assessment of the patient’s nursing care needs includes consideration of biophysical status, psychosocial status, age specific needs, cultural needs, environmental/discharge needs, spiritual needs, educational needs, advance directives, and the possibility of abuse or neglect.



Equipment utilized in the unit/dept:

• ventilators in patients who are stable and chronically ventilator dependent

• Non-invasive positive pressure ventilation (NIPPV), CPAP, BiPAP

• Temporary pacemaker

• Hemodialysis, Ultrafiltration

• CADD MS3, CADD Legacy I Infusion Devices

• Invasive catheters for central venous pressure monitoring

• Invasive catheters for arterial pressure monitoring

• Intracranial external ventricular drainage systems

• Intracranial pressure monitors

• charting is currently a mix of computerized and paper charting; new EMR implementation planned 8/4/12, transition to all computerized charting.
