Licensed Clinical Social Worker - LCSW

Harney District Hospital of Burns, OR is looking to hire a full-time LCSW to work with HDH primary care providers to assist patients with behavioral needs and support. This position starts at $35.15 and increases based on experience. You will provide an active presence to the HDH Family Care clinical teams and be available to accept warm-hand-off's and provide assessment and brief treatment interventions to patients referred by the medical providers. You will facilitate referrals and coordination to other services (e.g. substance abuse treatment, tele-psychiatry, or community resources) as needed or as directed by primary care provider. You will develop and maintain professional relationships and act as a liaison with the local mental health agency to enhance coordination of patients being referred for services at their facility. Your work will help patients effectively resolve individual barriers through follow-up, advocacy and collaboration with staff and other community service providers.

PREFERRED REQUIREMENTS: Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse audiences; Social and cultural sensitivity appropriate to ethnically and economically diverse patient and employee-base. Experience with motivational interviewing techniques.

Education and Experience

MANDATORY REQUIREMENTS: Master's degree in social work and / or counseling from accredited school.

SPECIAL REQUIREMENTS/LICENSES: Must possess an active and unencumbered Oregon State Licensed Clinical Social Worker (LCSW) license and / or Professional Counseling (LPC) license and / or Licensed Marriage and Family Therapist (LMFT) license and must be in good standing with the applicable Oregon Licensing Board.
