Afternoon/Evening shifts, flexible hours
5 hour shifts, Monday - Friday

PRINCIPLE RESPONSIBILITIES
- Provides hands on personal care, performs simple procedures as an extension of therapy or nursing services, assists in ambulation or exercises and assists in administering medications ordinarily self-administered safely, as directed.
- Homemaker/Home Health Aide Plan of Care.
- Assists the Administrator/Clinical Manager in developing and implementing a realistic, goal-oriented Plan of Care.
- Complies with infection prevention and control policies and procedures.
- Honors client rights by demonstrating respect for the client, his or her privacy, and his or her property.
- Demonstrates competency with assigned tasks.
- Observes and records the client’s care, symptoms, emotional and physical status.
- Recognizes basic elements of body functioning and changes in body function that must be reported to supervisor.
- Recognizes and reports changes in skin condition.
- Recognizes emergencies and the knowledge of instituting emergency procedures and their applications.
- Performs light housekeeping duties according to written Plan of Care. Maintains a clean, safe, and healthy environment for the client.
- Reports pertinent observations and significant changes in client’s condition to the Administrator/Clinical Manager promptly.
- Participates in client care conferences as directed.
- Maintains current and accurate client records, documenting care or services furnished.
- Submits all documentation to office according to dictated time constraints. Follows the Agency’s Documentation of Services policy.
- Responsible for monitoring for and reporting any suspected abuse to the supervisor immediately.
- Participate in Agency QAPI program as instructed by Administrator/Clinical Manager.
- Performs other duties as assigned.
What’s required?
- Certified Nursing Certification
- Must be 18 years of age or older
- Valid driver’s license and proof of auto liability insurance coverage
- BLS certification required but may be obtained after hire
- COVID-19 vaccine compliance
- Ability to lift, carry, push, or pull up to 50 pounds
