Job Title: Registered Nurse (RN) -

Overview: As a Registered Nurse (RN) at our rural hospice company, you will play a pivotal role in providing compassionate and skilled nursing care to patients facing terminal illnesses. Working in a rural setting, you will collaborate closely with interdisciplinary teams to ensure that patients and their families receive holistic and personalized care that aligns with their goals and wishes. Your role will encompass clinical expertise, emotional support, patient advocacy, and education for both patients and families. You will work in diverse environments, often within patients' homes or in hospice facilities, requiring adaptability and a commitment to delivering high-quality end-of-life care in rural communities.

Responsibilities:

  1. Patient Assessment and Care Planning:
  • Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
  • Develop individualized care plans in collaboration with the interdisciplinary team and in accordance with the patient's preferences and goals.
  • Regularly reassess patients' conditions and adjust care plans as necessary.
  1. Symptom Management:
  • Provide skilled nursing care to manage pain and alleviate symptoms associated with terminal illnesses.
  • Administer medications and treatments as prescribed, ensuring proper documentation and adherence to protocols.
  • Educate patients and families on symptom management techniques and medication management.
  1. Emotional Support and Counseling:
  • Offer emotional support and counseling to patients and their families throughout the end-of-life journey.
  • Foster open communication and provide a safe space for patients and families to express their concerns and fears.
  • Facilitate discussions about advance directives, end-of-life wishes, and goals of care.
  1. Patient and Family Education:
  • Educate patients and families about the hospice philosophy, disease progression, and available resources.
  • Guide caregiving techniques, including wound care, nutrition, and mobility assistance.
  • Offer bereavement support and resources to families both during the patient's illness and after their passing.
  1. Collaboration and Communication:
  • Collaborate effectively with the interdisciplinary team, including physicians, nurse practitioners, social workers, chaplains, and volunteers.
  • Maintain clear and timely communication with team members regarding patient status, care plans, and changes in condition.
  • Coordinate care transitions and ensure continuity of care across settings, including home, hospice facilities, and hospitals.
  1. Documentation and Compliance:
  • Maintain accurate and thorough documentation of patient assessments, care plans, interventions, and outcomes.
  • Ensure compliance with regulatory standards, including Medicare/Medicaid guidelines and hospice regulations.
  • Participate in quality improvement initiatives to enhance patient care and organizational processes.

Qualifications:

  • Current RN license in the state of practice.
  • Minimum of 2 years of nursing experience, preferably in hospice, palliative care, or a related field.
  • Strong clinical assessment and critical thinking skills.
  • Compassionate and empathetic approach to patient care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and collaboratively within a team.
  • Willingness to travel within rural communities and work flexible hours as needed.
  • Basic computer skills for documentation and communication purposes.
