REPORTS TO: Program Manager

CLASSIFICATION: Non-exempt, Full Time 

COMPENSATION: $30/Hr. plus eligible for benefits including: medical, dental, vision, life insurance, paid holidays, sick time, 401K plan

Agency Description:

Amigos de Guadalupe fulfills its mission by focusing on four strategies: Expansive Outreach (alcance comunitario), Trusted Information & Referral ( amigos de confianza), Targeted Direct Services Program (comunidad fortalecida) and Case management (compañerismo). The Case Manager will be mostly focused on comunidad fortalecida and compañerismo .

Founded on the principles of justice and advocacy, Amigos de Guadalupe Center for Justice and Empowerment is a founding member of the Si Se Puede Collective, a group of local agencies working together to make social services more accessible to the Mayfair Community; to strengthen individuals and the community through education; and to address social inequities by creating strong leaders through community organizing.

Agency and Casitas Initiative Background:

Based in Mayfair, the poverty experienced by the families served by Amigos is daunting. Most Mayfair residents work in low-wage jobs and are unable to access government aid due to their immigration status. The majority of households have inadequate food and poor overall nutrition. Additionally, due to the extreme lack of affordable housing, it is not uncommon for entire families to live in a single bedroom or garage. Amigos de Guadalupe’s Casitas de Esperanza (Tiny Homes of Hope) is a new tiny home community that allows for children and families to have a place to call home while they are in transition to permanent housing and self-sufficiency.

Position Summary: 

The Case Manager will conduct comprehensive community member assessments, evaluate their needs, and create both short and long term housing goals, with the ultimate goal of finding permanent housing for the community member. The case manager will also screen, place, and case manage individuals utilizing our core value of Radical Hospitality and looking through a 2-Gen approach lens. This position requires a high level of independent decision making, action, and responsibility, with an overall goal of enhancing the community member's quality of life by moving them out of poverty and into self-sufficiency. 

Essential Functions:

  • Provide case management services to unhoused community members who are placed in the Program: this includes goal setting, long-term case plan development and consistent support for the community member/family unit.
  • Champion money management guidance and tenant education to families.
  • Maintain a caseload of 50 families per year (average caseload is 20-25 families at any given time).
  • Meet with community members weekly or bi-weekly for 12 months and track their progress.
  • Connect community members with needed and available essential resources. 
  • Follow-up with community members/agencies to document use/success of referrals.
  • Support with coordination of service providers and volunteers for the site. 
  • Establish/input accurate and complete data for all contact with community members into the agency database.
  • Provide all required documentation in a timely manner which may include community member follow-up, outcome evaluations, and community member contact sheets.
  • Plan and coordinate health related educational events with health providers.
  • Willing to work weekend and night shifts.
  • Serve food for families on site as assigned by the Program Manager.

Other Functions:

  • Participate in regular staff meetings, staff training programs, supervisory meetings or any agency event.
  • Adhere to agency policies and procedures.
  • Perform any other duties as assigned.

Qualifications and Skills:

BA in Social Work

Must be fluent in Spanish

Minimum 2 years experience working in nonprofit setting

Previous experience with case management of families

Knowledge of community resources and counseling

Excellent documentation, communication and written skills

Ability to work in a variety of settings with culturally diverse individuals/families

Ability to lift 25lbs

Experience in working with the homeless population
