Circle of Life KOLA Home Care is growing, and we are looking for a nurse to join our Oglala, SD team. Circle of Life is a leading home care agency serving South Dakota PCA, SFC and nursing clients. We provide services for seniors and people with physical or mental disabilities.

The LPN will provide quality nursing care to clients in a home care setting under the direction and supervision of a Registered Nurse. The LPN will provide and promote activities ensuring the client & needs are met in an honorable manor, sometimes overseeing or assisting personal care staff in the care plan.

Responsibilities and Duties:

Open new clients by performing a verbal health status assessment via face-to-face encounter. Perform necessary nursing tasks in accordance with the care plan, continually evaluating the clients condition and collaborating with the care team.

Perform all nursing functions in accordance with established policies & procedures of the agency, current licensure, and acceptable standards of care. Perform additional nursing tasks such as charting and maintaining client files. Communicate with clients regarding scheduling and attending home visits. Provide supervision of personal care assist/caregivers for our clients. Maintain confidential information and upholds HIPAA regulations (training available). Promotes Circle of Life by attending community events, marketing services available to the community; collaborates with case managers and referral sources. Promotes compliance to participating state program. Participate in improving performance as rules and guidelines change. Participates in audits as needed.

Required Qualifications:

Degree or certification from an accredited college or technical school with one or more years of related experience and/or training. Knowledge of computer systems including Microsoft suite with an emphasis on Outlook, SharePoint, Excel, and Teams. Open to learning opportunities in programs available for billing, charting, and assessing clientele (Carevoyant, Therap). Skilled at working independently with minimal supervision. Prior experience in home care or general knowledge of home care operations. Able to maintain confidentiality with sensitive employee information. Ability to manage time and work with deadlines. Current licensure.

Preferred Qualifications: Experience working in a home care organization. Current CPR, First Aide

Position Type/Hours of Work:

This is a part time to full time hourly position. Hours are generally worked Monday-Friday but could include very occasional holidays or weekends. Hours may vary week to week and are dependent on nurse set scheduling.

Physical Requirements:

Must be able to lift 15 pounds at a time. Need to have reliable transportation and the ability to travel in a vehicle to drive to satellite office locations and client residence. Mileage will be covered based on nursing time and distance travelled.

Salary: $24.00 - $26.00 per hour
